Client Forms

All clients must print and fill out the Client Medical History Form and the Policies and Procedures Form and are asked to please bring these with you to your appointment. Covid 19 Waivers will be sent via email upon booking of your appointment and must be submitted electronically before each appointment. However, if you do not have access to receiving texts/electronic communications, in addition to all other forms, print the COVID -19 Waiver form and bring with you to your appointment. 

Laser clients must print and fill out the Client medical history, Policies and Procedures and ALL Laser forms (consent and skin type worksheet form) and please arrive with them for your appointment. 

Laser Pre and Post Treatment forms should be printed and reviewed and kept by clients for future reference.

Additional forms may be required for advanced treatments and will be provided to you upon arrival for your appointment.
